Reports to: Home Manager/Deputy
Scope of the post: The assessment of residents' needs and the development, implementation and evaluation of care plans. The post holder will be regularly in charge of a shift and will be expected to supervise and instruct staff to ensure the safe and efficient running of the home.
Principal Duties and Responsibilities
Liaise with parents, carers and relevant professionals to devise care plans which reflect the physical, psychological and spiritual needs of the resident. Implement and evaluate care plans.
Develop links with all residents’ relatives, care agencies and professional staff to facilitate seamless care delivery.
Organise regular care reviews and attend meetings pertaining to individual patients.
Manage your own time and that of others effectively through delegation to ensure a high quality of care.
Participate in an on-call rota as a point of contact and support for nursing colleagues.
Assist in developing and maintaining good working relationships within the home.
When delegated by the management team record and arrange cover for staff sickness.
To support the home manager and their deputy in the implementation of their role.
In the absence of the home manager or their deputy manage any complaints.
Promote collaborative working partnerships and effective communication between all members of the multi-disciplinary team.
Ensure that all clinical incidents/near misses within the sphere of responsibility are managed in a timely manner. Report any action taken to the home manager or their deputy to minimise risk.
Maintain confidentiality at all times
Minimise hazards in the working area and report identifiable hazards to the home manager or their deputy
Undertake risk assessments for each resident with other members of the team.
Promote awareness of Safeguarding referring any concerns to the home manager or Herefordshire safeguarding team.
Act as a mentor for all staff
Participate in the staff reviews, performance development and preceptorship process.
Undertake other duties and responsibilities as required in discussion with the Management.
Attend all in house and external training as required
Deliver training to care staff as directed by the home manager and training officer
Undertake specialist roles liaising with colleagues across Hereford and advising the home managers and other staff as required Professional Requirements
Follow the NMC Code of Practice.
Conform to the Post Registration Education and Practice requirements.
Follow the principles of Professional Practice
Maintain a portfolio of professional development for revalidation.
Ensure Personal Index Number (PIN) is valid.
